It's not glamorous. It doesn't have a TikTok following the size of niacinamide's, and nobody's turning it into a $60 "glow serum." But the naturally occurring dicarboxylic acid — found in wheat, rye, and barley, and produced synthetically for pharmaceutical use since the 1980s — has one of the more consistent clinical trial records of any topical ingredient sold over the counter.
What azelaic acid actually does in the skin
Azelaic acid works through several overlapping mechanisms rather than one dominant pathway, which is part of why it shows up across such different conditions. It inhibits tyrosinase, the enzyme that drives melanin production, but selectively — it targets hyperactive melanocytes in damaged or inflamed skin more than it affects normal pigment cells, which is why it doesn't bleach skin the way hydroquinone can. The strongest head-to-head data comes from a set of trials comparing 20% azelaic acid cream against standard acne treatments. Over-the-counter formulations top out around 10%, which is enough for maintenance and mild concerns but noticeably gentler than the 15–20% prescription strengths used in most of the trial data above. Start with an every-other-night application for the first two weeks regardless of which concentration you're using — azelaic acid causes a mild tingling or warming sensation on application for most people, and while it's rarely a sign of real irritation, working up to nightly use lets your skin adjust without the flaking that comes from going straight to daily use on retinized-looking skin.
